1. Introduction
Chronicles Psychiatric Services (“we,” “us,” “our”) is a professional mental health practice providing psychiatric care for children (age 6 and older), adolescents, adults, and seniors at our office in Duluth, Georgia. Our services encompass comprehensive psychiatric evaluations, medication management, individual psychotherapy, and family consultations, addressing conditions such as anxiety, depression, bipolar disorder, obsessive-compulsive disorder (OCD), attention-deficit/hyperactivity disorder (ADHD), trauma-related disorders, and other mental health concerns.
By accessing our website, scheduling an appointment, or receiving services, you (“you,” “patient,” or “legal guardian”) agree to be bound by these Terms and Conditions. These Terms supplement our Informed Consent Form and HIPAA Privacy Notice, which will be provided and must be acknowledged prior to treatment.
2. Scope of Services
Our in-person services include:
- Initial psychiatric evaluations (typically 45–60 minutes).
- Medication management follow-up visits (typically 15–30 minutes).
- Evidence-based psychotherapy sessions (typically 45–50 minutes), utilizing modalities such as cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT), eye movement desensitization and reprocessing (EMDR), and trauma-focused CBT (TF-CBT).
- Family consultations as clinically appropriate.
All services are delivered exclusively by board-certified psychiatric mental health nurse practitioners, led by David Sogbamu, CRNP, PMHNP-BC, in compliance with Georgia state regulations and professional standards.
3. Patient Eligibility and Responsibilities
To receive services, patients must:
- Be at least 6 years of age; minors require consent from a parent or legal guardian.
- Provide accurate and complete information regarding medical history, current medications, allergies, and relevant psychosocial details.
- Arrive punctually for appointments and provide at least 24 hours’ notice for cancellations or rescheduling.
- Adhere to prescribed treatment plans, including medication regimens and therapeutic recommendations.
Noncompliance may result in suspension or termination of services.
4. Appointment Scheduling and Cancellation Policy
Appointments may be scheduled via our website (chroniclespsychiatricservices.com), by telephone, or in person. Walk-in appointments are accommodated when possible.
Cancellations or rescheduling require a minimum of 24 hours’ advance notice. Late cancellations or failure to appear (“no-shows”) may result in restrictions on future scheduling. Repeated violations may lead to discontinuation of care.
5. Care of Minors (Ages 6–17)
A parent or legal guardian must accompany the minor to the initial evaluation and remain involved in treatment planning unless clinically contraindicated. Minors capable of understanding will provide assent to treatment. Limits of confidentiality, including mandatory reporting of suspected abuse or imminent harm, will be clearly explained.
6. Confidentiality and HIPAA Compliance
We protect all protected health information (PHI) in strict accordance with the Health Insurance Portability and Accountability Act (HIPAA) and Georgia privacy laws. PHI will not be disclosed without written authorization except as required for treatment, payment, healthcare operations, or by law (e.g., imminent risk of harm, abuse reporting, or court order). 7. Office Policies and Conduct
- Location and Parking: Services are provided at 2615 Peachtree Industrial Blvd, Suite J, Duluth, GA 30097, with complimentary parking available directly in front of the suite.
- Check-In: Patients should arrive 10 minutes prior to their scheduled time.
- Accompaniment: Only essential caregivers are permitted in treatment sessions.
- Recording: Audio or video recording of sessions is prohibited without prior written consent from all parties.
- Conduct: Respectful behavior toward staff and other patients is required. Disruptive, threatening, or violent conduct will result in immediate termination of services and possible involvement of law enforcement.
8. Termination of the Therapeutic Relationship
Either party may end the provider-patient relationship with reasonable notice. We reserve the right to terminate services immediately in cases of:
- Threatening or violent behavior.
- Repeated no-shows or cancellations.
- Ethical or clinical conflicts.
Upon termination, we will facilitate referrals to alternative providers and assist with the transfer of medical records, subject to applicable legal requirements.
9. Emergency and Crisis Protocol
Chronicles Psychiatric Services does not provide emergency or crisis intervention services. In the event of a psychiatric emergency:
- Dial 911 or proceed to the nearest emergency department.

- Utilize the Georgia Crisis and Access Line: 1-800-715-4225.
10. Limitation of Liability
While we strive to deliver high-quality care, psychiatric treatment outcomes cannot be guaranteed. We shall not be liable for indirect, incidental, or consequential damages.
11. Governing Law and Dispute Resolution
These Terms and Conditions are governed by the laws of the State of Georgia, without regard to conflict of law principles. Any disputes arising hereunder shall first be addressed through mediation in Gwinnett County, Georgia. If mediation fails, disputes will proceed to binding arbitration under the rules of the American Arbitration Association.
12. Amendments to Terms
We reserve the right to modify these Terms with 30 days’ prior notice, posted on our website and in our office. Your continued use of services following such notice constitutes acceptance of the revised Terms.
